Our Home
We're located within a shared space called The Connection at 560 S. Perryville Rd, Suite 5, Rockford, Illinois.
Our building is in the parking lot of Sam's Club in-between Movement Fitness and Murphy's Pub. We're not a standalone building so know that there can be noise from other businesses within the building. Know that we've done everything we can to keep you comfortable for your service.



Booking Policies

We do require a valid credit or debit card on file at the time of booking.

We do keep a card on file for your convenience at check out and for any missed appointment fees or cancellation fees that may apply.

We highly recommend booking for next appointment in person before you leave to lock in a time that works.

Our booking system will send an appointment reminder and confirmation via Text and Email. By booking your appointment, you agree to receive your notifications this way.

If you're a new client, you'll receive a link to your client portal in your email before the next business day with access to your intake forms.
Cancellation Policies

We have a 24-hour cancellation policy that applies to all clients.

In the case that you need to reschedule your appointment prior to 24-hours before, please notify us or use the confirmation email to reschedule to avoid late cancellation fees.

If less than 24- hour notice given we reserve the right to charge your card on file 50% of your service fee for your appointment and missed income.

If no call/no show for your appointment, or if you reschedule/cancel two appointments in a row, you may be asked to pre-pay for your service prior to rescheduling.
Arrival Time

If you're a new client, please fill out our intake forms that were sent via your client portal prior to arrival if possible.

If our suite door is closed, please wait for our previous client to be completed.

Please arrive at your appointment time or no earlier than 5 minutes, unless if you need to fill out your intake forms, which no more than 10 minutes needed.

If you know you're running late, please text or call to let us know you're on your way. We will try to fit as much of your scheduled service into your appointment time as possible.
Corrective Skin Clients

New corrective skin clients start with a consult or consult and beginning treatment.

Client may be directed to schedule a Telehealth visit with our medical director.

If corrective care clients fail to follow aftercare instructions then provider may terminate treatment.
Lash Extension Clients
-
Lash extension clients will need to have routine lash fill appointments.

If go beyond 3.5 weeks for a fill then can be charged for full set of lashes.

If clients fail to follow aftercare instructions of cleaning or pick at lashes resulting in more than 50% of lashes needing to be replaced, then can be charged for a full set.

Order Processing and Payment
We accept major credit cards and other secure payment methods. Orders are subject to acceptance and availability. We reserve the right to cancel or refuse any order for any reason.
Returns and Refunds
We accept returns of unopened products within 30 days of delivery. Products must be unused and in their original packaging. Contact customer service to initiate a return. Shipping fees are non-refundable.
